Job Summary:
The Resource Nurse is responsible for serving as a home health clinical expert and mentor for staff demonstrating high standards of care delivery, modeling professionalism, answering questions, and supporting client care/ programmatic needs. The Resource nurse provides clinical education support through competency checkoffs, clinical consultation and demonstrates effective communication with case managers and the interdisciplinary team to ensure the provision of compliant high-quality care and services. They act as the lead mentor for job shadows and student learner experiences, providing valuable learning opportunities through observation of their daily work environment, responsibilities and interactions with patients/families and the care team. The resource nurse serves as a visit nurse and provides support/coverage of case management as needed while continuing to develop and deepen their own knowledge and skills related to nursing best practice, case management and care coordination for clients in the home health setting.
Education and Licensure
• Current RN license to practice professional nursing in the State of Vermont is required.
• Associate degree in nursing required; Bachelor of Nursing Degree preferred.
• 4 years of relevant clinical experience preferred, with at least 2 years of home health experience required.

• Ability to work in an autonomous care setting.
• Organized and able to manage multi-tasking in a fast-paced environment.
• Demonstrates cultural competence by respecting and understanding the diverse backgrounds and beliefs of patients and families.
