Job Summary
Odyssey House Louisiana, Inc. is seeking a full-time Registered Nurse for the Community Health Center. The schedule is Monday-Friday 8am-5pm. This role is a full-time grant funded position by the Hepatitis C Elimination Initiative Pilot. The RegisteredNurse (RN) provides and coordinates patient care for participants in theagency's Hepatitis C (HCV) grant program within the Federally Qualified HealthCenter. This role offers direct clinical support, including monitoring patientprogress, facilitating access to HCV treatment, and providing education onprevention, treatment options, and overall liver health. Reporting to the NurseManager, the RN is responsible for developing and implementing individualizedcare plans in collaboration with physicians, social workers, and otherhealthcare professionals to ensure seamless, high-quality care. The RN alsoserves as a patient advocate, helping participants navigate barriers totreatment, empowering them to make informed health decisions, and supportingthem throughout their HCV care journey.

Duties & Responsibilities
It is the duty and responsibility of the Registered Nurse to:
- Assesses patients' conditions, monitor vital signs, administer medications and treatments, and ensure patient safety
- Develops and implement individualized care plans, collaborating with other healthcare professionals to ensure comprehensive and effective patient care
- Educates patients and their families about health conditions, treatment options, and preventative care. They also advocate for patients' needs and ensure their voices are heard
- Work closely with physicians, therapists, social workers, and other healthcare professionals to coordinate patient care and ensure a smooth transition of care
- Administers medications, including injections, IVs, and oral medications, ensuring accuracy and safety
- Monitor patients' responses to treatment, assess their progress, and adjust care plans as needed
- Prepares to respond to emergencies, providing immediate care and support to patients in critical situations
- Stay up to date on the latest advancements in healthcare, participate in continuing education, and maintain their professional certifications
- Document all pertinent health related information about clients on the patient chart that is done routinely and according to nursing standards of practice
- Complete all incident reports before the end of the scheduled shift if applicable; and
- Maintain HIPAA guidelines; and
- Perform other duties as assigned.
MINIMUM QUALIFICATIONS
- CPR certification, first aid training, and experience in specific areas like emergency or critical care nursing helpful
- Ability to work collaboratively in a multidisciplinary setting
- Proficient in electronic health records
PREFERRED QUALIFICATIONS
- Associate's Degree in Nursing, Bachelor of Science in Nursing, Nursing Diploma or equivalent
