About the Role
We are seeking an Advanced Practice Provider (APP) provides reasonable and prudent medical care, practices appropriate management, and uses sound clinical judgment based on best-practice standards and evidence-based medicine. The SCRC/RWC APP provides medical services within the scope of their licensing, training, and credentialing in collaboration with the Behavioral Health team for patients with various forms of addiction.
Key Responsibilities
• Provides reasonable and prudent medical/surgical care, practices appropriate management, and uses sound clinical judgment based on best practice standards and evidenced based medicine.
• Leads Medical services at SCRC and RWC, taking responsibility for clinical quality, operational efficiency, and patient centeredness at these site(s).
• Coordinates with Behavioral Health and nursing colleagues, and collaborates with other specialties.
• Educates patients on preventive health care and chronic disease management or refers to appropriate resources or agencies within the confines of SCRC and RWC protocols, which are dictated by the BH department.
• Consults with other staff or specialists to make appropriate referrals for acute, chronic and preventive health care.
• Enters appropriate legible notations and documents encounters appropriately. Completes notes, forms and coding in a timely manner.
• Participates in training as required by the Department Medical Director.
• Meets all licensing, certification, and other requirements as set forth by state, federal, professional, hospital, accrediting and health payer entities.
• Actively pursues continuing medical education opportunities to maintain one's knowledge and skills.
• Performs other related duties as assigned.
Required Qualifications
Education: Degree (Nurse Practitioner or Physician Associate) from an accredited Nursing or Physician Associate school. CPR Certification required.
Experience: Board Certification or Board Eligibility. (Board certification must be attained within the timeframe dictated by one's specialty in order to remain on staff at the CS-HHC if one was originally Board Eligible at the time of one's hire.) Connecticut Licensure. Must have at least 5 years of clinical experience in outpatient internal medicine or family practice. Prior experience working in addiction medicine preferred.

It provides 24/7 inpatient and ambulatory detox services, outpatient addiction treatment, mental health care, and integrated medical services.
RWC: 149 Minor St, New Haven, CT 06519
RWC in New Haven, CT refers to the Recovery and Wellness Center, a newly opened 31,000-square-foot behavioral-health and addiction-treatment facility operated by the Cornell Scott-Hill Health Center. It provides intensive outpatient services, primary medical and dental care, and a short-term therapeutic living environment for individuals with substance-use disorders and co-occurring mental-health conditions.
