Job Description
- Position Summary
- The Oncology Navigator assumes responsibility and accountability for the management of resources through interdisciplinary collaboration necessary and appropriate for the achievement of optimal patient care outcomes for oncology patients. Work is performed with general independence and decision making responsibility in accordance with clinical guidelines and scope of practice. Navigation in cancer care refers to individualized assistance offered to patients, families and caregivers to help overcome barriers to care and facilitate timely access to quality medical and psychosocial care, from diagnosis through all phases of care and survivorship. Maintain accurate and concise record of patient observations, treatments, and procedures administered and documents the findings in the medical record and survivorship care plans. Maintains master document of survivorship care plans for breast patients, documenting and monitoring due dates for the plans. Meets with patient to present care plan, documents when plan is presented by other providers and ensures that care plans are provided to all breast cancer patients. Navigator must have clear written and verbal communication skills, self-direction, and organizational skills.

- Associates in Nursing - Required
- Experience
- 2-3 Years Clinical experience or related field in the past 5 years - Required
- License & Certification
- Professional License with State of Practice - Required
- Oncology Certification (OCN) or other Breast Navigator certification preferred. Employee expected to obtain certification when eligible to sit for exam.
- Basic Life Support (BLS) - Required
- Core Job Functions
- Follows national guidelines and best practices to navigate cancer patients along a disease continuum of care. Facilitate communications between the managing physicians, patients and diagnostic and treatment facilities. Facilitate continuum of care navigation by scheduling and assuring patient compliance with treatment plan.
- Assesses the quality and effectiveness of interdisciplinary, intra-agency, and inter-agency communication and collaboration. Establishes collaborative relationships within and across departments that promote patient safety, culturally competent care, and clinical excellence. Develops processes for attaining and sustaining Nurse Sensitive Indicators and other quality metrics outperformance of identified goals.
- Adheres to the release of patient information and is compliant with HIPAA regulations. Verifies identification and copies of patient information release for records and billing purposes.
- Utilize nursing process per the scope and standards for oncology nursing practice
- Facilitates physicians and staff involvement in developing new processes and organizing operations that enhance emerging practice standards. Develops, maintains and reports on clinical quality dashboards that are supported from multiple databases. Provides updates on nationally mandated cancer quality initiatives and draws correlations with current practices and the NCCCN guidelines.
- Documents, completes, and disseminates survivorship care plans for breast cancer patients. Maintains master document of plans, documenting and monitoring due dates for the plans.
