- Position Summary
- The RN project manager is responsible for supervision and training of care coordinators and support staff for a caseload of elderly or disabled persons through the St. Joseph/Candler CCSP program. The position requires community development and identification of resources in counties served. Internal policy must be developed and maintained. The project manager must be a liaison with state policy makers and contribute to policies as they are developed. The position will require a person with leadership and organizational skills.

- Additional Experience Information - Registered nurse in one of the following areas. Two years in the field of Geriatric nursing, community health, long term care, chronic diseases of adults
- License & Certification
- Professional License with State of Practice - Required
- Core Job Functions
- Actively participates in the development and implementation of CCSP statewide policies and procedures, performance and improvement initiatives, provider networks, and regulatory issues. Develops relationships with AAA, DCH and GMCF staff.
- Interprets state policies and procedures. Implements and updates policies as developed internally or as mandated by the department of community health. Includes oversight of QI and initiating operational changes with staff.
- Provides and /or arranges initial training and orientation for new staff. Ensures on going in-service, educational training for staff to include development of service plans, chart documentation, formal/informal relations, primary care, and supportive services.
- Works to increase overall project enrollment. Provides identification and education of potential referral sources, manage wait lists, and referral process. Acts as liaison for support service providers to strengthen relationships.
- Statistical data is accurate, compiled, and submitted on a monthly basis.
- Responsible for monitoring state requirements for standards of promptness for documentation, client contacts, and up loads to GMCF
