Summary:
- To provide physical therapy treatment, under the directions and supervision of a licensed physical therapist, to patients receiving home care services through Penn Medicine at Home (PMAH).
Responsibilities:
- Receive patient clinical and scheduling information through the supervising PT or team scheduler
Review and follow the PT treatment plan and individual patient goals. Be aware of any precautions set forth by the physician or supervising PT
Correctly interpret the instructions for patient treatment set forth by the physical therapist. Appropriately advance the home exercise program and therapeutic activities to improve function.
Display compassion and empathy in all patient interactions. Maintain an accurate schedule on the documentation software.
Documents all contact with patient/family/payors or referral sources in road notes.
Specific knowledge of other therapy disciplines and services provided (occupational, and speech therapy).
Demonstrates proficiency in required software systems including road notes and outlook.

- Education Specialization: CAPTE certified physical therapy assistant program Equivalent Experience:
