We are seeking an LPN for consistent evening coverage for this private duty nursing case.
Responsibilities
Monday through Friday, the participant attends outpatient rehabilitation. The assigned nurse accompanies the participant and remains available to:
The nurse is not expected to provide physical, occupational, or speech therapy services. The nurse's responsibility is to provide physician-ordered skilled nursing care while the participant attends rehabilitation.

- Medically stable
- Neurogenic bladder
- Home and outpatient rehabilitation setting

We are seeking an LPN for consistent evening coverage for this private duty nursing case.
Responsibilities
- Perform ongoing nursing assessments.
- Administer medications as ordered by the physician.
- Perform intermittent catheterization per physician orders.
- Monitor urinary status and document urine output.
- Monitor skin integrity and implement pressure injury prevention measures.
- Assist with safe transfers, positioning, and bed mobility.
- Monitor the participant's condition and promptly report changes to the physician and agency.
- Complete timely and accurate electronic nursing documentation.
- Communicate effectively with the participant, family, physician, rehabilitation staff, and Penn Home Health Care clinical team.
- Follow the physician's orders, nursing plan of care, and agency policies at all times.

- Perform physician-ordered skilled nursing interventions as needed.
- Administer medications as ordered.
- Perform intermittent catheterization when due.
- Monitor the participant for changes in condition while under the nurse's care.
- Communicate with rehabilitation staff regarding nursing concerns when appropriate.
- Document all skilled nursing services provided.
The nurse is not expected to provide physical, occupational, or speech therapy services. The nurse's responsibility is to provide physician-ordered skilled nursing care while the participant attends rehabilitation.
