We are excited to spotlight Xtalks Featured Community Member Sarah Ropp, HBSc, AEMCA, ACP, ADS, CP, Specialist, Paramedic Education at Peel Regional Paramedic Services, whose work brings together prehospital clinical research, frontline paramedic education and operational leadership. She shares her experience managing randomized controlled trials in emergency care, developing high-fidelity simulation training and her vision for strengthening paramedicine through research that can directly inform patient care. 

What Are Your Proudest Life Science Achievements?

Among my proudest achievements is the work I have led in human randomized controlled trials, some of the most rigorous and impactful research that can be conducted in the life sciences. At Peel Regional Paramedic Services, I have had the privilege of simultaneously managing four active human randomized controlled trials: PITSTOP, EpiDOSE, DOSE VF and OPTIVO. Each trial represents a significant scientific undertaking, requiring meticulous research design, research ethics board engagement, regulatory compliance, data integrity oversight and seamless cross-agency coordination, all within the dynamic and unpredictable environment of prehospital emergency care.

What makes these achievements particularly meaningful is the real-world impact they carry. These are not laboratory-based studies; they involve real patients, real paramedics and real outcomes. Perhaps most significantly, the DOSE VF trial has produced findings that have changed how defibrillation is delivered to patients in cardiac arrest, not just here in Peel or across Ontario, but across Canada and around the world. To have played a role in research that has reshaped global clinical practice is something I carry with enormous pride.

Achieving the highest patient enrollment rate among all participating paramedic services in the PITSTOP trial, securing the second-highest enrollment in EpiDOSE and being selected as the lead paramedic service for the OPTIVO trial are further recognitions that reflect not just my own dedication, but the extraordinary commitment of the teams I have had the honor of leading and supporting.

Equally significant is the work I have done to prepare and train more than 715 frontline paramedics in mass casualty incident (MCI) response and incident management systems (IMS). Developing this training was not simply a matter of delivering content. It required building high-fidelity, simulation-based scenarios that authentically replicated the chaos, complexity and human stakes of a real mass casualty event.

Every scenario was carefully constructed to challenge paramedics’ clinical decision-making, communication under pressure and execution within a command structure, ensuring that when a real incident occurs, our people are not just prepared; they are confident, coordinated and ready to lead.

What Are Your Day-to-Day Job Highlights?

No two days look the same, which is honestly one of the things I love most about my role. On any given day, I might be overseeing the progress of an active research trial, mentoring a paramedic through a new protocol, developing training content or working through the operational details of a large-scale education initiative.

As Acting Supervisor of Education & Research, I have also had the opportunity to contribute to leadership decisions that shape how our service grows and evolves. But the moments I find most rewarding are the quieter ones, when a paramedic gains confidence in a new skill or when a research milestone is reached that brings us one step closer to better patient outcomes. 

What Are Your Favorite Life Science Innovations?

Point-of-care research in paramedicine is something I find truly exciting. The idea that we can generate meaningful clinical evidence in real time, in the field and with real patients is remarkable.

The growth of simulation-based and technology-enhanced education, including tools such as Articulate and immersive learning platforms, has also transformed how we train paramedics, and I have embraced that fully in my own practice.

As someone who has worked in community paramedicine, I am also deeply passionate about innovations that move healthcare upstream by using data, technology and community integration to prevent crises before they happen rather than simply responding to them. 

What Is Your Vision for the Future of the Industry?

My vision for the future of paramedicine is grounded in the power of prehospital research and immersive, high-fidelity education. Human randomized controlled trials have already begun to reshape what we know about prehospital care, and I believe we are only scratching the surface of what this research can accomplish.

The DOSE VF trial stands as a powerful example of what is possible: research conducted by paramedics on the front lines that has fundamentally changed how defibrillation is delivered to patients in cardiac arrest, not just locally, but across Canada and globally. That is the standard I believe our industry should aspire to, science that does not sit on a shelf, but travels across borders and saves lives.

The evidence generated through trials such as PITSTOP, EpiDOSE and OPTIVO has the potential to continue that legacy, directly influencing clinical protocols, improving patient survival rates and firmly establishing paramedicine as a leader in applied life sciences research on the world stage.

At the same time, I believe deeply that knowledge is only as valuable as our ability to transfer it. The future of paramedic education must be immersive, scenario-driven and built on the principles of high-fidelity simulation experiences that do not just teach procedures, but also develop the critical thinking, situational awareness and leadership skills that define exceptional paramedics.

Whether preparing a crew for a mass casualty incident or training the next generation of research-engaged clinicians, the standard we set in education today will define the quality of care patients receive tomorrow.
