Penn Home Health Care is seeking a compassionate and dependable Licensed Practical Nurse (LPN) to provide one-on-one skilled nursing care for an adult participant in the home setting in Kennett Square, PA.

This position offers consistent scheduling, competitive pay, and the opportunity to provide individualized care in a supportive home environment.
Compensation

• $35.00 per hour

Schedule

• Morning shifts
• 8-hour shifts available
• 12-hour shifts available
• Flexible scheduling
• Full-time, part-time, and per diem opportunities available

Responsibilities

• Provide one-on-one skilled nursing care in the participant's home
• Administer medications and treatments as ordered
• Monitor medication effectiveness and observe for adverse reactions
• Monitor blood glucose levels and diabetic status as ordered
• Perform ongoing nursing assessments and report changes in condition
• Monitor skin integrity and implement pressure injury prevention measures
• Assist with bowel and bladder management, including incontinence care as ordered
• Monitor for signs and symptoms of infection or other complications
• Assist with safe transfers and positioning using appropriate equipment
• Perform or assist with range of motion exercises as ordered
• Reinforce participant and caregiver education
• Communicate changes in condition promptly to the RN Supervisor and physician
• Maintain accurate and timely nursing documentation
• Promote participant safety, dignity, comfort, and independence

Qualifications

• Current Pennsylvania Licensed Practical Nurse (LPN) license in good standing
• Current CPR certification
• Reliable transportation
• Strong assessment and communication skills
• Ability to work independently in the home setting
• Home health, rehabilitation, long-term care, or private duty nursing experience preferred
