High-Level Findings
Consulting and Speaking Fees
Many payments appear to camouflage inappropriate remuneration. In 2023, there were over 6,600 instances where companies made multiple payments to providers for non-educational events. This is despite regulatory warnings about repeat payments.
Food and Beverage Payments
Industry guidelines suggest that food and beverage payments should be modest and not part of entertainment events. However, 2023 saw a peak in high-dollar food and beverage payments and group events.
Honoraria Payments
Contrary to federal guidelines, repeat honoraria payments reached more than $33 million in 2023. These payments are supposed to be for brief, one-time activities, yet patterns suggest otherwise.
Impact on Scientific Integrity
Drug and device makers funding researchers also made $2.3 billion in parallel cash and in-kind payments since 2016. This dual funding threatens the integrity of scientific research, raising concerns about bias and conflict.

The Open Payments Program
The Centers for Medicare and Medicaid Services (CMS) established the Open Payments program in 2013. The program aims to enhance transparency in financial relationships between healthcare providers and the pharmaceutical and medical device industries. The database includes details on payments for consulting, speaking, honoraria and in-kind payments.
